Nursing Facility Medicaid Cost Report Reporting Form Now Available

According to a Minnesota Department of Human Services communication, the reporting form for the 9-30-2010 Nursing Facility Medicaid Cost Report is now available on the provider portal.

The due date for submitting a completed report is January 15, 2011.  A 15-day extension to this deadline can be requested through December 15 by sending an e-mail to DHS.NFRP.CostReport@state.mn.us.

Changes made to the instruction manual are shown in underline (for additions) and strikeout (for deletions).
