Your 2011 Membership Packets are being sent at the end of this week. Each membership (not campus) will receive a packet labeled “Important Membership Materials.” Packet contents include:

• 2011 Membership Directory. This handy little book will help you quickly connect with colleagues, identify vendors of products and services you need, and find answers to questions about Care Providers of Minnesota (CPM), the American Health Care Association (AHCA), the National Center for Assisted Living (NCAL), and more.
• Although we strive to include accurate and up-to-date information, we apologize for any misprint or incorrectly-listed information. To update any of your information included in the Membership Directory, please contact Lynn Tierney at 952-854-2844 or at ltierney@careproviders.org. Any updates will be reflected in our online Membership Directory immediately (found in the “Just for Members” section of our website: www.careproviders.org) and will be included in the 2012 Membership Directory.

• Contact Information Confirmation Form (if applicable). This form will help us keep our records current. (Note: You may not receive this form if your establishment is connected to another membership in our system.) We ask that you look over the staff list, add/remove any people that have joined/left your organization, update titles, email addresses, spelling mistakes, etc. There is also a column where you can indicate if each person should be receiving our Action Newsletter and other Email Alerts directly from Care Providers of Minnesota. Simply fax or mail in the form upon completion.

• Education and Region Meeting calendar. These calendars are tentative, as programs may be added throughout the year and dates are subject to change. Be sure to watch your mail, email, and newsletter for the most up-to-date information on educational offerings.

• Year Tab for your Membership Plaque. Simply remove the old year tab and slide this one in. If you do not have a Membership Plaque and would like to get one (they are free), please contact Jason Espersen at 952-851-2497 or at jesperse@careproviders.org.

• 2011 Membership Decal(s) to be attached on the inside of your front door/window.

• Loads of information about Care Providers of Minnesota’s staff, programs, and national affiliates: The American Health Care Association (AHCA) and the National Center for Assisted Living (NCAL).
