Help us reach our $15,000 fundraising goal by golfing in the 2011 Care Providers of Minnesota Foundation Annual Golf Tournament. The tournament is the Foundation’s largest source of ongoing support for its programs, including scholarships and Leading4Life. Last year, with your help, the Foundation was able to award scholarships to 25 deserving students. Fourteen fellows are participating in the 2011 Leading4Life program, with Foundation funding providing important program assistance.

By participating as a golfer, by inviting a colleague or peer to join you this year, by suggesting that one of your business partners sponsor a hole this year, or simply by making a donation if you can’t join us, you will help ensure the continuation of these vital programs.

The 2011 Golf Tournament will be held July 19 at Crystal Lake Golf Club in Lakeville. Participants will tee off at 11:00 a.m. A reception and awards presentation will follow the tournament; you’ll have a chance to visit with previous scholarship recipients and Leading4Life recipients and hear how important the Foundation has been in their careers. The cost, which includes greens fee, cart, range balls, tee packs, lunch, and the reception is $150 for nursing facility/assisted living/senior housing members. Non-sponsoring Associate Members and prospective member fees are $190. A portion of this is tax-deductible.
