The Minnesota Department of Human Services (DHS) requests that nursing facilities that receive payments for Medical Assistance clients residing in a single room due to medical necessity begin using two new forms intended to make the process more efficient and streamlined.

Effective immediately, DHS will only be accepting two forms for single room authorization and resident status changes.

• There is a ONE-page single room authorization form to use for single room authorization requests, and this form needs to be sent directly to Mr. Leahy either by email or fax.
• Also, there is a ONE-page resident status form to notify Mr. Leahy of any resident status change (i.e., death, discharge).

To download the single room authorization form and the resident status form, please go to the Just for Members > Payment Sources > Medicaid Billing section of the Care Providers of Minnesota website.

In order to get caught up, Mr. Leahy would like facilities to send him a single room request using the new single room authorization request form for any pending authorizations that they have, regardless of whether or not the nursing facility already has sent in anything to DHS. Nursing facilities will need to indicate the effective date of the single room on the bottom of the form.
