The Minnesota Department of Human Services has announced that the reporting form for the 9-30-2011 Nursing Facility Medicaid Cost Report is now available on the provider portal at https://nfportal.dhs.state.mn.us.

According to the Instruction Manual for the Annual Statistical and Cost Report of Nursing Facilities:

• The deadline for submission of the Annual Report is 11:59 p.m. on January 17, 2012.
• The department may grant facilities one extension of up to 15 days for filing of this report, if the extension is requested by December 15, 2011. Extension requests must be submitted in writing or via email to Bev Milotzky. Extension requests will not be granted via the telephone. Extension requests via email may be sent to Bev Milotzky at: DHS.NFRP.CostReport@state.mn.us.
• If approved for an extension request you will receive a confirmation email indicating that your due date is 11:59 p.m. on February 1, 2012.
