The Minnesota Department of Health (MDH) has posted RUG-IV Consumer Fact sheets on the MDH Case Mix webpage at http://www.health.state.mn.us/divs/fpc/profinfo/cms/RUGIVindex.html.

According to MDH, nursing facilities may provide copies of the fact sheets to residents, resident representatives and family members who wish additional information about case mix, a specific RUG-IV classification or how to request a reconsideration.
